This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

This Notice of Privacy Practices describes how we may use and disclose your protected health information to carry out your treatment, collect payment for your care, and manage the operations of this clinic. It also outlines our policies concerning the use and disclosure of this information for other purposes as permitted or required by law. Additionally, it details your rights to access and control your protected health information (PHI), which includes demographic information that may identify you and relates to your past, present, or future physical or mental health or condition, and related healthcare services.

We are required by federal law to abide by the terms of this Notice of Privacy Practices. We may change the terms of our notice at any time. The new notice will be effective for all protected health information we maintain at that time. You may obtain revisions to our Notice of Privacy Practices by emailing us at drklayton@anywherechiropractic.com, calling the office, and requesting a revised copy, or asking for one at your next appointment.

A. Uses and Disclosures of Protected Health Information

 

By applying to be treated in our office or at your location (house calls), you imply consent to the use and disclosure of your protected health information by your doctor, our office staff, and others involved in your care and treatment for the purpose of providing healthcare services to you. Your protected health information may also be used and disclosed for billing purposes and to support the operation of the practice if Anywhere Chiropractic LLC ever becomes an insurance provider.

Uses and Disclosures Based on Implied Consent

 

  1. Treatment: We will use and disclose your protected health information to provide, coordinate, or manage your healthcare and related services.
  2. Payment: If Anywhere Chiropractic LLC becomes an in-network insurance provider, we may use your protected health information as needed to obtain payment for services provided.
  3. Healthcare Operations: We may use your protected health information to support business activities such as quality assessment, employee training, or educational purposes.

If we become an in-network insurance provider, we may share your protected health information with third-party “business associates” (e.g., billing or transcription services). We will have written contracts with these associates to ensure your information is protected.

We may also contact you with appointment reminders, information about treatment alternatives, or other health-related benefits and services that may be of interest to you.


B. Your Rights

 

You have the right to:

  • Inspect and obtain a copy of your protected health information.
  • Request a restriction on how your information is used or disclosed.
  • Request confidential communication through alternative means or at an alternative location.
  • Request amendments to your protected health information.
  • Receive an accounting of certain disclosures of your protected health information.
  • Be notified of any breach of your information.
